THE CITY OF FRESNO AND NORTH FRESNO PRIMARY CARE, INC. (BAUTISTA
MEDICAL GROUP) REGARDING COVID-19 TESTING
THIS GRANT AGREEMENT (AGREEMENT) is made and entered into effective
upon execution by both parties (the Effective Date), by and between the CITY OF
FRESNO (the CITY), and NORTH FRESNO PRIMARY CARE, INC. (Bautista Medical
Group)(GRANTEE) to provide funding for COVID-19 contact tracing and quarantine
support.
RECITALS
WHEREAS, in light of the COVID-19 global pandemic, and particularly its effect on
underserved communities in the City of Fresno, there is a need for prevention education,
contract tracing and medical investigation, and quarantine support; and
WHEREAS, GRANTEE shall provide the services set forth in the attached Scope
of Services related to prevention education, contract tracing and medical investigation,
and quarantine support, including needs assessment and referral services; and
WHEREAS, GRANTEE represents it desires to and is professionally and legally
capable of immediately performing the services set forth in this Agreement for City of
Fresno residents; and
WHEREAS, this program is a necessary expenditure occasioned by the COVID-
19 public health emergency, not accounted for in the City's budget as of March 27, 2020;
and
WHEREAS, GRANTEE acknowledges that grant funds being provided under this
Agreement will be derived from the City's Coronavirus Relief Fund allocation under the
federal CARES Act (42 U.S.C. 801 et seq.) and subject to the constraints set forth in the
CARES Act; and
WHEREAS, this Agreement will be administered for the City by its City Manager
or its designee.
AGREEMENT
NOW, THEREFORE, in consideration of the foregoing and of the covenants,
conditions, and premises hereinafter contained to be kept and performed by the
respective parties, it is mutually agreed as follows:
1.Scope of Services. GRANTEE shall perform to the satisfaction of the CITY
the services described in Exhibit A, including all work incidental to, or necessary to
perform, such services even though not specifically described in Exhibit A.
2.Grant Amount. City shall provide GRANTEE an amount not to exceed
$1,040,000 for the services described in Exhibit A. The City shall render payment to in
three separate installments as set forth below:
Phase 1: City will pay GRANTEE a first grant installment of up to $520,000 for the
Project described in Exhibit A. The City's Phase 1 payment will be due on the Effective
Date of this Agreement.
