SECOND AMENDMENT TO AGREEMENT
THIS SECOND AMENDMENT TO AGREEMENT (Second Amendment) made and
entered into as of September 1, 2023, amends the Agreement entered into between the
CITY OF FRESNO, a municipal corporation, (City), and Halcyon Behavioral
(CONSULTANT)

WHEREAS, the City and the Consultant entered into an agreement, dated September 1,
2020, for professional Employee Assistance Program services for the City Employees
(Project); and
WHEREAS, the City and the Consultant desire to extend the Agreement to August 31,
2026; and
WHEREAS, the Consultant agrees it has no claim, demands, or disputes against the City.
AGREEMENT
NOW, THEREFORE, the parties agree that the aforesaid Agreement be amended as
follows:
1. The recitals to this Amendment are incorporated and made a part of this
Amendment.
2. The Agreement is extended to August 31, 2026.
3. Except as otherwise provided herein, the Agreement entered into by the City and
the Consultant on September 1, 2020, and amended on September 12, 2021,
remain in full force and effect.
