THIS FIRST AMENDMENT TO AGREEMENT(First Amendment) made and entered into
as of September 1, 2021, amends the Agreement entered into between the CITY OF
FRESNO, a municipal corporation, (City), and Halcyon Behavioral, a California
Corporation (Consultant)
RECITALS
WHEREAS, City and Consultant entered into an agreement, dated September 1, 2020,
for professional Employee Assistance Program services for the City Employees
(PROJECT), and
WHEREAS, the City and the Consultant desire to extend the Agreement to August 31,
2023, and
WHEREAS, Consultant agrees it has no claim, demands, or disputes against City.
AGREEMENT
NOW, THEREFORE, the parties agree that the Agreement be amended as follows:
1. The recitals to this Amendment are incorporated and made a part of this
Amendment.
2. The Agreement is extended to August 31, 2023.
3. Except as otherwise provided herein, the Agreement entered into by City and
Consultant on September 1, 2020, remains in full force and effect.
