THIRD AMENDMENT TO
GRANT AGREEMENT BETWEEN THE CITY OF FRESNO AND UCSF FRESNO
REGARDING COVID-19 TESTING, TRACING, AND QUARANTINE SUPPORT
THIS THIRD AMENDMENT TO GRANT AGREEMENT is effective as of April 23,
2021, and amends the Grant Agreement between the CITY OF FRESNO, a municipal
corporation (CITY), and UCSF FRESNO (GRANTEE).
RECITALS
A. CITY and GRANTEE entered into the Grant Agreement effective July 29,
2020, and First Amendment effective November 19, 2020, and Second
Amendment approved by Council March 4, 2021 (collectively, the
Agreement), related to COVID-19 testing, tracing, and quarantine support
(Mobile HeaL).
B. CITY and GRANTEE now desire to modify the Agreement by increasing the
funding as set forth below.
AGREEMENT
NOW, THEREFORE, in consideration of the above recitals, which recitals are
contractual in nature, the mutual promises herein contained, and for other good and
valuable consideration hereby acknowledged, the parties agree that the Agreement
be amended as follows:
1. Compensation. GRANTEE shall be paid an additional amount not to exceed
$310,000 for services to be rendered under the Agreement. Of that amount,
$100,000 shall be allocated immediately to insure little or no disruption of service.
2. Effect of Amendment. Except as expressly modified by this Third Amendment,
the Agreement is hereby reaffirmed and ratified. In the event of any conflict
between the Agreement and this Third Amendment, this Third Amendment shall
control.
