PURPOSE
The purpose of this policy is to provide instruction on how to complete and submit an
FD-19 for the Fresno Fire Department (FFD or Department) Construction and
Fabrication Program.
APPLICATION
This form is used to submit project proposals for the Construction Fabrication
Program.
OPERATIONAL POLICY

OPERATIONAL GUIDELINE
The Construction Fabrication Request Form (FD-19) should be completed by
members for project proposals that meet the criteria of the Construction Fabrication
Program.
PROCESS
Completing the FD-19
1. Select project type (Training or Station/other).
2. Describe proposal in a few sentences.
3. List benefits to FFD.

4. Include materials list .
5. Provide cost estimate.
6. Route the FD-19 to the Deputy Chief of Training for review. If
approved, the originator of the proposal will be advised and will work
with the Shop Steward and the Construction/Fabrication Cadre to
complet e the project. If the project is denied, the completed FD-19 will
be routed back to the originator.
7. The s hop steward will review the FD-19 for completeness and viability.
He/she should confirm the proposal being considered is not pending
through another process (i.e., capital improvement request through
Operations).
8. The shop steward will confirm project costs and establish a project
timel ine, should it be approved. Additional space is provided for
comments.
