PURPOSE
This policy provides information for completion and submission of a position
application for a 300/400/500 level position for review and consideration
APPLICATION
300/400/500 level position applications are reviewed at the agency/department level
and consideration for qualification is determined by the fire chief
OPERATIONAL POLICY
This form is to be filled out by members who meet a 300/400/500 level position as
identified by Firescope and Administrative Manual, Section 103.004, California
Incident Command Certification System (CICCS). Upon completion of the form and
applicable documentation, forward the information to the chief officer responsible for
Office of Emergency Services (OES) responses.
OPERATIONAL GUIDELINE
Members submitting this application should confirm that they meet the requirements
set forth by the Fresno Fire Department (FFD or Department) in Administrative
Manual, Section 103.004, California Incident Command Certification System
(CICCS) and the current position qualification guide.
PROCESS
Complete the form with pertinent information to include course completion
information.

Currently held trainee positions.
Currently held qualified positions.
Record of assignment experience.
Once form is filled out forward the following supporting documentation to the
Department OES chief:
Copy of certificates of completion of class(es).
Copy of current “Red Card”.
Other pertinent and supporting documentation that proves qualification.
Once received and reviewed by the Department OES chief, the packet is forwarded
to the fire chief for signature.
An email is sent to the Operational Area OES Coordinator, the Regional review
committee or the State review committee. If approved by committee a letter is sent
to the Department and Fresno Emergency Command Center (ECC) for updating
Resource Ordering and Status System (ROSS).
An updated red card will be produced to reflect the qualification change (s).

1. Red Card: Incident qualification card issued by the Training Division which
lists fully qualified and trainee status. This card can be found in LMS within
each members credentials
