PURPOSE
Administrative Manual, Section 106.067, Rapid Intervention Crew Checklist, FD-66
provides a guideline of minimum equipment, size-up, and information to be gathered
by the Rapid Intervention Crew (RIC).
APPLICATION
The FD-66 is designed to provide a guide to minimum equipment, size-up, and
information to be gathered by the RIC at any time a RIC is assigned to an incident.
Copies of this form along with a pen are located in the pocket of the RIC Air Pack.

OPERATIONAL GUIDELINE
The company officer assigned to the RIC may utilize the FD-66 as a guideline of
minimum equipment, size-up, and information to be gathered at an incident. The
FD-66 is a guideline to optimize RIC actions any time a RIC is assigned to an
incident.

PROCESS
Size Up:
Building Type and Dimensions: List building construction type (heavy
timber, ordinary construction).
List fire building dimensions including the
number of stories (LxWxH).
Building Occupancy: List building occupancies.
Collapse Potential: Estimate the potential for collapse based on
building construction, fire involvement,
length of time subjected to burn, and other
pertinent factors.
Access/Exit Points: Identify firefighting access points and check
for exit points. If insufficient exit points
exist, modify the existing exits or add exits
to the structure.
Roof Type: List roof type and construction (lightweight
truss with shake, bowstring).
Ladders (Two): If there are roof operations on the fire
ground ensure there are at least two (2)
properly deployed ladders to each roof.
Fire Ground Tactics:
Offensive/Defensive: Identify current tactics and initial tactics.
For example, “Offensive mode with interior
attack.”
Time of Alarm: Time of initial alarm.
Time First Unit Entered: Time first interior attack/investigation crew
entered the structure.
Elapsed Time: Total time from time of alarm to now. For
example, “1806 hours to 1820 hours – 14
minutes.”

Entry Crews and Entry Location: List crews on the interior. This information
should be available from the IC. Also note
the time at which this information is current.
Roof Crews: List crews on the roof. Also note the time at
which this information is current.
Accountability Initiated: Confirm that collection of passports and
personal accountability report has been
initiated.
Minimum RIC Equipment:
Stokes Litter: Use the Stokes litter to carry equipment and
mark the RIC tool cache.
Carryall: A carryall is an effective tool for victim
removal and is light weight and
maneuverable. It can also be used to wrap
up the victim and the RIC pack when exiting
the building.
RIC Pack (Air/Mask): RIC pack includes search rope bags and
RIC checklist.
Thermal Imager: Bring this item.
Haligan/Flat Head Axe: Bring this item.
Narrow Beam Light at Entry Point: Incidents have shown a narrow beam light
placed at the door can provide a beacon,
which a lost member(s) may be able to
identify in low visibility situations.
Chainsaw: Use on ordinary construction to create or
enhance exits.
Circular Saw with Metal Blade: Use on metal doors or bars to clear exits for
interior crews.
Life Rope: Utilized for removal of firefighter from a hole
or can also be used to extend a search
rope. Tie knots at 25-foot intervals and
obtain a carabiner.

Location of Missing Firefighter: List the last known location of the missing
firefighter.
Unit (Company of Firefighter): List the unit of the missing firefighter. For
example, Engine 2 or Truck 1.
Name: Identify the missing firefighter by name.
Assignment: Write the missing firefighter’s last known
assignment.
Radio Channel for FF Missing: Identify the channel dedicated to the
missing firefighter.
Fire Ground Sketch:
Place a rough fire-ground sketch in this box or place a larger sketch on the back of
the paper, include ladders, entrance exit locations, and ABCD side designators as
identified by the incident commander.
