106.036 COMMUNITY OUTREACH EVENT FORM (FD-18)

PURPOSE
To provide documentation of events conveying fire prevention training, education to
the public and use of Fresno Fire Department (FFD or Department) facilities.
APPLICATION
This form is to be used to document interaction with the public at planned and
unplanned events where fire prevention training and education is provided, station
tours are conducted, and the public use of a fire station community room. The form
may be filled out upon request for Department interaction as a scheduled event, or
filled out upon completion of the event. This form should be filled out electronically
through the entry portal Pub. Ed. /FCO found in the FireRecords favorites menu or
by following this link http://vs-ch-fireapp2/fd18_form/index.asp .

OPERATIONAL GUIDELINE
The Community Outreach Event Form (FD-18) is to be completed whenever an
appropriate request is received for the Department to respond to a community event,
education program, school carnival or fair, block parties, station tour, public use of a
fire station community room, etc. An event scheduled for a fire station or fire
company should be added to the station calendar on the date requested.

PROCESS
All information on the form is to be completed when the Department is requested at
an appropriate public education event, a station tour is scheduled or a request is
made to use the fire station community room.
The FD-18 is initiated by the appropriate fire station captain or fire prevention
inspector responsible for the event. Upon completion of the event, electronic entry
will be finalized by the members attending the activity. If the paper form is used, it
should be routed to Fire Prevention for filing. Forwarding of the paper form is not
required if an electronic entry has been made.
