Johnson, Senior Administrative Clerk Section 106.035 Page 1 of 4 PURPOSE The purpose of this policy is to explain the Administrative Manual, Section 106.035, Probationary Member Evaluation Report Short Form (FD-17B) that is completed at the end of the evaluation period for probationary members of the Fresno Fire Department (FFD or Department). OPERATIONAL GUIDELINE Form FD-17B is a short evaluation form designed to capture a probationary member’s performance on a short-term or limited basis. This form is to be completed and routed to the primary officer’s assigned station before the next calendar day after the evaluation period. A short-term evaluation period is defined as any length of time between 8 and 24 hours in which the officer is not the primary evaluating officer. If an officer working with a probationary member for less than 8 hours observes member performance that warrants documentation, he/she can request a form FD-17B from the member. The officer then documents the member’s performance and routes the completed form to the primary evaluating officer. A simple sentence is sufficient to document standard performance. Section 106.035 Page 2 of 4 The supervisor shall discuss the report with the member being evaluated. No copies of the report will be retained by the rating officer or intermediate officers. Short forms may be routed with the primary long form evaluation. The FD-17B shall be reviewed by the Training Chief and forwarded to the Fire Chief. It will be placed in the member’s personnel file at headquarters. The short forms are to be reviewed along with the long form by the responsible officer receiving all forms. The report shall be received in Administration no later than the 15th day of each month following the rating period. Evaluation Techniques The following techniques and suggestions are offered to aid supervisors in arriving at a well-considered evaluation. Allow plenty of time, free from interruption, for completing the evaluation. Base evaluation on the member’s actual performance, rather than on what rater thinks the member can do. Each evaluation should be made after considering the member’s work during the entire period covered by the report. It is not intended for use as a form of disciplinary action. It should be noted this is a probationary evaluation report, which should indicate the member is to be graded on probationary status and not as fully-trained and experienced member. Actual length of service should be considered. PROCESS From the Administrative Manual, open the form or complete the hard copy delivered by the probationary member If completing the form in Word Doc format, use the mouse or TAB key to move around the document. Do not use the ENTER key. Note: Some of the cells have been blocked to avoid revisions. Enter the member’s name adjacent to the heading MEMBER: (Last, First, MI). Enter the assignment in which the evaluation occurred. Enter the date the evaluation was completed and the period the evaluation covered. If there are criteria rater wishes to comment on, an “X” shall be placed in the appropriate box and comment in the evaluator’s comments section. If the criteria are non-applicable, do not check the box. Print the evaluation form. Forward completed form to primary evaluating officer. Section 106.035 Page 3 of 4 How to Mark the Evaluation Report The member’s performance is to be rated on each of the ten criteria. The criteria are indicated on the FD-17B and should be interpreted according to their descriptions. Note: Engineers are not to be evaluated on report writing, administrative ability, or supervisor ability. If an evaluating officer wishes to comment on one of the criteria, check the adjacent box and comment in the space provided. If more space is needed, an additional sheet of paper can be used and stapled to the form. If during the evaluation period no observations were made in a given area, a simple statement indicating this is necessary.
