HomeMy WebLinkAbout106.039 - FD-22, Shift Replacement-Overtime Form (SR-OT) (FD-22) ADMINISTRATIVE MANUAL PAYROLL AND FINANCE 106.039 SHIFT REPLACEMENT / OVERTIME FORM (SR/OT) (FD-22) EFFECTIVE: OCTOBER 1, 2008 Current Revision Date: 04/21/21 Next Revision Date: 04/21/26 Author’s Name/Rank: Mike Getty, Business Manager Review Level: 1 Administrative Support: Evelyn Yin-Zepeda, Management Analyst II PURPOSE This policy provides clarification how and when to complete and submit the Shift Replacement/Overtime Form (SR/OT) (FD-22). APPLICATION The FD-22 is used to verify correct hours are paid and is to be submitted as backup documentation when applicable. OPERATIONAL POLICY The electronic FD-22, SR/OT Worked Form, will be filled out for all partial SR/OT in the Department staffing software program. Field Battalion Chiefs and 40-hour Supervisors are to submit a completed form prior to leaving the work assignment. OPERATIONAL GUIDELINE Supervisors should verify the times worked and ensure accurate documentation in the Department staffing program. Personnel working OFF ROSTER should, individually, enter hours worked into the Department staffing software program. A Training Division or 40- hour Supervisor should verify the times and approve the requested overtime hours prior to the export of Payroll. The amount of overtime worked should be calculated and entered in 6-minute increments to begin with the first 6 minutes of the hour. An Overtime Report submitted as a result of an off-duty recall is limited to only those hours a member is assigned to work. No allowance for travel time to the assigned duty station is permitted. Members wishing to credit overtime worked to accrued compensating time (CTO) off should notify their respective battalion chief prior to 2000 hours, on the day of the CTO accrual. Note: Accrual of CTO is not allowed for overtime worked as UHR, CTR, or UV. PROCESS 1. The member working overtime is responsible for the accuracy of times entered into the Department staffing software program. If times need adjustment, the member should contact their respective battalion chief to make the change in the system. 2. Battalion chiefs will verify the overtime worked and complete the electronic FD- 22 in the Department staffing software program for 56-hour members.
