SECTION 106.001 QUALITY IMPROVEMENT FORM (EMS-01)
PURPOSE
The purpose of this document is to establish responsibilities and procedures for the
reporting of and response to quality improvement reports for the Fresno County EMS
Communications Center.
APPLICATION
To best meet Fresno Fire Department (FFD or Department) operational needs, issues
involving the Fresno County EMS Communications Center are to be addressed using
the form EMS-01; Fire Dispatch Continuous Quality Improvement (FDCQI).
OPERATIONAL POLICY

OPERATIONAL GUIDELINE
Members who identify issues involving the Fresno County EMS Communications
center should complete the form and forward it though the identified channels for
review and response.

Save the document as a PDF and e-mail it to the Fire Department Communications
Liaison.
The document is reviewed by the Fire Department Communications Liaison, who can
research the issue and add Originating Agency’s Liaison Findings and Comments.
If the issue cannot be resolved by the Fire Department Communications Liaison, the
document will be forwarded by the liaison to the Responding Agency’s Liaison for
review and response.
The document is to be reviewed by Fresno County EMS Communications Center
personnel and the findings and comments are to be provided.
The document is to be returned by the Responding Agency’s Liaison to the Fire
Department Communications Liaison for review. The liaisons will work together to
ensure a satisfactory resolution. This may result in continued education training or
policy change.
The completed document is then forwarded by the Fire Department Communications
Liaison to the originator for review.

DEFINITIONS
1. Fire Department Communications Liaison/Originating Agency Liaison:
A member serving as a link between the Department and the Fresno
County EMS Communications Center. This is an administrative
assignment determined by the Fire Chief per Administrative Manual,
Section 101.001, Administrative Assignments.
2. Responding Agency’s Liaison: Fresno County EMS Communications Center
personnel assigned to communicate and coordinate activities between the two
entities.
