106.034b PROBATIONARY ENGINEER EVALUATION
REPORT SHORT FORM (FD-17B)
EFFECTIVE: SEPTEMBER 2008
Current Revision Date: 1/25/19 Next Revision Date: 1/25/21
Author’s Name/Rank: Jonathan Chew, Deputy Fire Chief Review Level: 2
PURPOSE
To provide instructions on completing a Probationary Engineer Evaluation Report
Short-Form (FD-17B).
APPLICATION
The FD-17B is a short-term progress report for members in a probationary engineer
status and is to be completed by company officers who observe a probationary
engineer in the work environment for any length of time between 8 and 24 hours.
OPERATIONAL POLICY
The probationary engineer will be evaluated by a supervising officer by the end of
the evaluation period and routed via Fresno Fire Department (FFD or Department)
mail to the primary company officer’s assigned station.
OPERATIONAL GUIDELINE
The FD-17B is a short evaluation form designed to capture probationary engineer
performance on a short-term or limited basis. This form is to be completed and
routed to the primary company officer’s assigned station before the next calendar
day after the evaluation period. A short-term evaluation period is defined as any
length of time between 8 and 24 hours in which the company officer is not the
primary evaluating officer. If a company officer working with a probationary engineer
for less than eight hours observes member performance that warrants
documentation, he/she can request a form FD-17B from the member. The company
officer then documents the member’s performance and routes the completed form to
the primary evaluating company officer. A simple sentence is enough to document
standard performance.

The supervisor is to discuss the report with the member being evaluated. No copies
of the report are to be retained by the rating officer or intermediate officers. Short
forms are to be routed with the primary long form evaluation.
PROCESS
The FD-17B is to be received by the Training Division no later than the tenth day of
each month following the rating period. The report will be reviewed and signed by
the Deputy Chiefs in the Training, Operations, and Personnel Divisions and the Fire
Chief; an executed copy will be placed in the member's personnel file at Department
headquarters.
Evaluation Techniques
The following techniques and suggestions are offered to aid supervisors in arriving
at a well-considered evaluation. Allow plenty of time, free from interruption, for
completing the evaluation. Base the evaluation on the member's actual
performance, rather than on what rater thinks the member can do. Each evaluation
should be made after considering the member's work during the entire period
covered by the report. It is not intended for use as a form of disciplinary action.
It should be noted, this is a probationary evaluation report. The member is to be
graded on probationary status and not as a fully-trained and experienced member.
Actual length of service should be considered.

5. Enter the date the evaluation was completed and the period the

6. If there are criteria rater wishes to comment on, an X will be placed in
the appropriate box and comment in the evaluator’s comments section.

How to Mark the Evaluation Report
The member's performance is to be rated on each of the 10 criteria. The criteria are
indicated on the FD-17B and should be interpreted according to their descriptions.
Note: Engineers are not to be evaluated on report writing,
administrative ability, or supervisor ability.
If an evaluating company officer wishes to comment on one of the criteria, check the
adjacent box and comment in the space provided. If more space is needed, an
additional sheet of paper can be used and stapled to the form.
If during the evaluation period no observations were made in a given area, a simple
statement indicating this is necessary. An example might read, “No observations
made or not observed”.
