PURPOSE
The purpose of this policy is to discuss the FD-55, Transfer of Command form,
which is a worksheet for the initial incident commanders (ICs) when multiple units
are responding to an incident.
APPLICATION
The FD-55 form is to be completed anytime the initial IC is addressing subsequent
responding units.

INSTRUCTION SHEET
Completing the Form:
The top left section denotes actions taken upon arrival at the scene. Anything
highlighted in gray shall be verbalized over the radio.
Report on Conditions (ROC): First in officers ROC from the cab.
Resource Needs: Three possible responses, “resources at scene suf-
ficient,” “standby,” or order additional resources.
360°: Verbalize a 360° has been completed or to whom it
has been assigned.
Savable Human Life: “YES”- The incident commander (IC) is beyond
reasonable suspicion that there is some-
one in the building AND the survivability
profile of the structure supports them still
being alive.
“NO”- Either, it has been confirmed through verifi-
able souraces that the building is all clear
OR the survivability profile will not support
life.
“UNKNOWN”- Not enough information is available
to make high risk rescue attempt.
As soon as resources arrive, a
search of the area will be conduct-
ed.
Operational Mode: The mode should be verbalized over the radio. De-
termination of the Operational Mode is based on
the application of the FFD Risk Management Policy
and may change with the conditions or because
certain benchmarks (i.e., all clear) are obtained.
See Standard Operating Procedures Manual, Sec-
tion 201.005, Risk Management Policy for further
information.
Strategic Objectives: Should be verbalized over the radio. Should be
succinct, achievable goals completed through tacti-
cal assignments.

The top right section denotes tracking of resources.
Listing of Resources Assigned: This includes space for a full commercial alarm as-
signment, 2nd alarm, and overhead positions.
Tactical Assignments: The most common tactical assignments are listed
with space to write the assigned unit. Additional
assignments can be written in below as needed. It
is strongly recommended that initiation and com-
pletion of assignments are verbalized and time
hacks noted in the space provided next to the tacti-
cal assignments.
Radio Channels: Note the Operations channel designator. Addition-
al space for a Command channel and TAC chan-
nels, if needed.
The bottom/back section identifies the incident location.
Address: Utilize to keep all responding apparatus oriented to
the incident location.
Incident Name: Designated by Communications and is the title of
the IC, e.g., “Dakota IC.”
Incident Map: Utilize to help orient the incident for the incoming
battalion chief.
