APPLICATION According to the American Heart Association (AHA), providing effective basic life support by ensuring a good airway and performing optimal ventilations and compressions will increase the likelihood of successful defibrillation. OPERATIONAL POLICY The Department will comply with all state and local policies in accordance with this policy. OPERATIONAL GUIDELINE This section intentionally left blank. PROCESS 1. Equipment: a. Zoll AED Pro Biphasic AED. b. Carrying case with internal equipment pocket. c. Defibrillator pads (2 sets required). d. Razor. e. 4” x 4” gauze pads (2). f. Alcohol wipes (2). Department-issued jump drive. 2. Service Messages: a. Green Check Mark - Self-test is completed and AED is ready for use. b. Red X - Unit not ready for use. c. Unit Failed text prompt - Unit not ready for use. d. Change Battery text prompt – Battery life is very limited; replace immediately. e. Low Battery text prompt - Battery life is at 50 percent. 3. Service and Battery Replacement: The pads are stored pre-connected to the AED. If a Red X appears in the service window, the most likely cause is a poor plug connection. Ensure the plug is securely connected. If the issue is not resolved, press the On/Off button. Once the AED is on, turn it off and recheck the window for the green check mark. If the unit is still not ready for use, consult the operator’s guide, Chapter 7, and notify the EMS coordinator. If the AED cannot be fixed immediately, contact a battalion chief or the EMS coordinator for a replacement. The AED batteries are disposable and have a limited shelf life. There will be no spare batteries available. If the “Low Battery” text prompt is visible on the display screen, the AED should still be safe to use; however, notify the EMS coordinator to have a new battery ordered as soon as possible. For a “Change Battery” text prompt, notify the EMS coordinator or a battalion chief for immediate AED replacement. 4. Daily Checks: The AED is to be checked daily to ensure it is ready for use. The AED will perform self-checks every seven days and every time the unit is turned on. The daily check shall include inspection of the following: a. A green check mark in the service window. b. A complete inventory. Effective Date: November 2009 Kevin Reynolds, Captain Current Revision Date: 05/09/2022 Section 317.021 Next Revision (2) Date: 05/09/2027 Page 3 of 6 c. Pad expiration date. d. Check that the battery life is at 50 percent or above. 5. CPR: Refer to Central California EMS Agency (CCEMSA) Basic Life Support (BLS) treatment Protocol 510.08 (CPR Protocol). 6. Sequence: Poor pad adherence or air pockets can cause arcing, skin burns, or reduced energy delivery. Apply only freshly opened, undamaged pads to clean, dry skin. Clip or shave any excess hair and dry any moisture from the chest prior to applying pads. Press and release the On/Off button; the unit will issue the message “UNIT OK” in voice and text prompt. If not already done, attach the pads to the patient’s bare chest according to package instructions. Once the pads are in place and the analyze button has been pressed, the AED will issue a voice and text prompt “DON’T TOUCH PATIENT, ANALYZING.” Note: The AED will not prompt you to check for a pulse after two minutes or allow time to do so prior to analyzing. This is a program setting, which cannot be overridden. It is allowable and safe to check for a pulse during the analyze phase, even though the voice prompt states, “DON’T TOUCH PATIENT, ANALYZING.” Do not touch the patient while pressing the SHOCK button. Prior to pressing, make sure all persons around the patient are clear. The AED will issue an alert tone when two minutes time has elapsed. To analyze, press the Analyze soft key. Continue sequence per CPR Protocol. Document any application and usage of the AED on a multiple-page Patient Care Report (PCR) (refer to CCEMSA Policy No. 812). Note: For pediatric patients one to eight years of age, the AED does not have child pads; place adult pads anterior and posterior. Effective Date: November 2009 Kevin Reynolds, Captain Current Revision Date: 05/09/2022 Section 317.021 Next Revision (2) Date: 05/09/2027 Page 4 of 6 7. Special Considerations: a. Termination of CPR: If the decision is made to terminate CPR efforts (refer to CCEMSA Policy No. 549 for criteria.). Document the reason for termination of CPR and the assessments on the multiple-page PCR (CCEMSA Policy No. 812). The fire crew must stay with the body until law enforcement, the ALS provider, or the coroner arrives, or the coroner releases the body to family members. b. Do Not Resuscitate (DNR) Documentation: i. If DNR documentation is presented, refer to CCEMSA Policy No. 564 prior to the start of CPR. Note: If there is any doubt regarding the criteria outlined in CCEMSA Policy No. 564, start CPR. ii. Document the type of DNR and the assessment on a multiple- page PCR (CCEMSA Policy No. 812). The fire crew must stay with the body until law enforcement, the ALS providers, or the coroner arrives, or the coroner releases the body to family members. c. Drowning: Refer to CCEMSA Policy No. 510.08 for drowning resuscitation guidelines. Remove the patient from standing water and dry the patient’s chest prior to AED use. d. Electrocution: Refer to CCEMSA Policy No. 510.08 for cardiac arrest guidelines due to electrocution, make sure the scene is secured from electrical hazards prior to application of AED. e. Pacemakers and Automated Internal Cardiac Defibrillators: Do not place pads over an implanted device. The pad may be angled to avoid placing it over the device. Data Storage: The AED will store up to four events. If data is not downloaded and the AED is used on a fifth code, the oldest event will be erased. It is strongly recommended to download information after each use. Electronic data from the AED will be transferred from the AED to the station computers via jump drive. Note: Use Department-issued jump drives only. Personal jump drives are not to be used. Using a personal jump drive may result in incomplete data transfer or deprogram the AED. If a jump drive is lost or damaged, replacements can be ordered through Supply. Enter into the AED non-rescue mode by pressing the On/Off button for five to ten seconds. The AED will issue the voice and text prompt “NON RESCUE MODE.” Plug the jump drive provided into the USB port located in the battery compartment and press the “USB” softkey under the display screen. Select the “all” softkey when prompted. The information will transfer to the jump drive and be removed from the AED. The AED will shut off automatically when the download is complete. It is now safe to remove the jump drive. The station computers have a ZOLL icon on the desktop, which is connected with the K drive for data storage. Log in using your personal login and password (same as Outlook). The Zoll RescueNet Data Retrieval program can connect to the K drive when opened. Plug the jump drive into the station computer and click on the ZOLL icon. After the program opens, click on the “USB” icon on the top banner of the screen. The information will automatically transfer into the program and be removed from the flash drive. You should be able to see the event information by clicking on the desired event. Click on the gray “General” tab and enter the fire incident number under the field labeled “Run Number.” No other information is required in this program. All other information, including a narrative, is to be entered into the Tiburon records management system. A copy of the PCR which includes all crew member names, certification numbers, and initials of the AED operator is to be routed to the EMS coordinator.
