PURPOSE
This policy provides the format and instructions for completing the FD-72 and FD-
72.1 forms for the Fresno Fire Department (FFD or Department).
APPLICATION
This section left intentionally blank.
OPERATIONAL POLICY
The FD-72 and FD-72.1 will be completed weekly during inspection of US&R
equipment and after each use. Information pertaining to the US&R equipment will
continue to be documented on the applicable FD-39 (SCBA), FD-57 (Chainsaw),
and FD-71 (Rope Log). The Company Officer is responsible for completing the
report for the assigned apparatus.
OPERATIONAL GUIDELINE
Forms are to be routed to the Shop through the Special Operations Battalion Chief
to be filed. Prior to submittal, the Company Officer is to enter the training in LMS
from the upper left portion of the FD-72. Training hours captured on this form should
count towards a member’s continuing education (CE).

1. The FD-72 and FD-72.1 are to be printed in landscape format. The applicable
shift and date is to be entered in the spaces provided on the form.
2. Items in operational ready state are to be indicated in the associated box.
3. All defects/deficiencies discovered are to be noted in the remarks section. An
additional sheet of paper may be attached to the report, if needed.
INFORMATION
Adhere to proper procedures as set forth in the Standard Operating Procedures,
Training & Equipment Manual, and the Administrative Manual regarding to
equipment requiring servicing, repairs, or replacement.
