PURPOSE
Administrative Manual, Section 106.066, Incident Tracking Log, FD-64 is a two-sided
form utilized to track members and equipment from one assignment to other
functional areas at large incidents.
APPLICATION
The FD-64 is used to augment the Passport Accountability System to track
personnel at large incidents.

OPERATIONAL GUIDELINE
While operating at complex incidents such as a high rise fire, supervisors/managers
assigned to key functional positions within the Incident Management System, should
use the FD-64 to track the movement of personnel through functional areas of the
incident.
PROCESS
Completing the Form:
The first side of the form is utilized to track personnel.
1. When crews receive an assignment and depart to a functional area,
their passport is returned to them.

2. The supervisor/manager is to document the functional area
represented on the space provided.
3. The supervisor/manager from the functional area departed is to log the
identifier of the crew, the number of members assigned to the crew,
the time of the request, the new assignment, and the time of departure.
(Example: Base to Lobby.)

4. As the crews pass through other functional areas, their movement is
documented. (Example: from Lobby to Staging.)

5. When the crews arrive at their assignment, they are documented on
the tracking log and their supervisor/manager obtains their passport.
Utilizing the incident tracking log enables the incident commander to determine the
last checkpoint a crew passed through departing to an assignment for accountability
purposes.
The DATE acronym is printed on the bottom of the form to ensure proper transfer of
information to crews departing for assignment.
The second side of the form is utilized to track equipment.
1. When equipment is requested by a supervisor/manager, the equipment
is documented on the equipment tracking log. The company identifier,
equipment type, quantity, request time, and arrival time are
documented.

2. When the equipment arrives at the functional area, the equipment is
documented on the Tracking Log.

Utilizing the incident tracking log enables the requesting supervisor/manager to track
equipment, eliminating the potential for double orders.
The equipment tracking log also assists the demobilization phase of the incident.
Knowing the amount of equipment removed from an apparatus allows for timely
restoration prior to being released from the incident.
