Form Number: FD-38
Title: PHYSICAL FITNESS PROFILE
Contents: A record of physical fitness evaluation results and fitness
goals of individual personnel.
Frequency: Semi-annually (six month intervals) - March/April and
September/October.
Responsibility: Company officers shall conduct the fitness evaluation and
record the results on the FD-38.
Channels Through: Member maintains the FD-38 and keeps in his/her
possession.
Remarks: The form is to be completed in pencil.

The physical fitness evaluation sha ll be conducted when a member begins the
fitness program and semi-annually thereafter. Evaluations will be administered by
the company officer.
The results of each member's fitness evaluation shall be recorded in pencil on the
FD-38. The company officer is responsible for the proper completion of the FD-38.
After each semi-annual evaluation, each member will be required to set goals the
member expects to attain by the next scheduled evaluation and indicate the goals
on the FD-38, adjacent to the results of the evaluation. Goals should be set at
realistic, attainable levels.
Ultimate goals, based on generally accepted standards for the fire service, will be
set by the Fitness and Health Committee and indicated in the Standard Operating
Procedures, Section 203.11, “Physical Fitness Program.”
Instructions for administering each element of the fitness evalu ation are contained in
the Standard Operating Procedures, Section 203.11, “Physical Fitness Program.”
Note: The "Recovery Heart Rate" is obtained by administering the
"Kasch Step Test". Reference Standard Operating Procedures
manual, Section 203.11, “Physical Fitness Program.”
"Waist Flexibility" measurements are to be entered as a minus
(-) if members do not reach toes, a zero (0) if member touches
toes, or a plus (+) if member passes toes with finger tips.
