APPLICATION The FD-17D is to be completed by the immediate supervisor for each probationary captain under his/her command. This requirement is to be met during the captain’s probationary appointment. OPERATIONAL POLICY The probationary captain will be evaluated by his/her immediate supervisor on the last scheduled working day of each month. The supervisor will discuss the report with the member being evaluated. OPERATIONAL GUIDELINE This section intentionally left blank. PROCESS The FD-17D is to be received by the Training Division no later than the tenth day of each month following the rating period. The report will be reviewed and signed by the Deputy Chiefs in the Training, Operations, and Personnel Divisions and the Fire Chief; an executed copy will be placed in the member's personnel file at the Fresno Fire Department (FFD or Department) headquarters. As indicated on the FD-17D, it is mandatory that ratings of “Below Standard” be addressed by a Performance Improvement Plan. This requirement should not dissuade the rater from providing an honest appraisal. Section 106.034d Page 1 of 9 Each evaluation is to be made after considering the member's work during the probationary period covered by the report. The supervisor should not base his/her judgment entirely on the member's work immediately preceding the evaluation period, or on isolated actions atypical of the member's work. It should be noted this is a probationary evaluation report, which will indicate the member is to be graded on probationary status and not as a fully trained and experienced member. Actual length of service should be considered. A rating of “Above Standard” does not indicate the member is superior to veteran members, the member is only superior in the probationary position. How to Complete the Evaluation Report Form 1. The FD-17D can be found at TargetSolutions (LMS) > File Center > Manuals, Procedures, and Forms > 100 Administrative Manual > 106.034d, FD-17D Probationary Captain Evaluation Report and on the City’s website at Services Tab > Fire Department > FFD Training Division > Policy, Procedures, Manuals, and Forms > 100 Administrative Manual > 106.034d, FD-17D Probationary Captain Evaluation Report. 2. Use the TAB key or mouse to move around the document. Do not use the ENTER key. Note: Some of the cells have been locked to avoid revisions. 3. Type member’s name adjacent to the heading Member: (Last, First, MI), not above the line. 4. Enter the month, day, and year of the evaluation. 5. Enter the station and shift the probationary member is assigned (i.e., E13A). 6. Enter the rating evaluator’s name. 7. Enter the dates of the evaluation period. 8. Enter which evaluation month the member is in (i.e., 1, 3, 7). 9. Answer each statement in the quality of information box with the appropriate numerical value. 10. The rating category boxes can be checked by either of the following methods. Section 106.034d Page 2 of 9 a. Highlight the box by using the TAB and pressing the SPACE bar. The space bar will toggle between checked and unchecked. b. Click the desired box. This will toggle between checked and unchecked. 11. The comments section and the Performance Improvement Plan section are text boxes. The box will expand to fit the rater’s needs. If more space is needed for documentation, an additional sheet of paper can be added. 12. The Member Comments section will be lined. Any captain wishing to comment on his/her evaluation can do so; however, he/she will have to handwrite his/her comments. 13. Print the evaluation form. How to Mark the Evaluation Report The member's performance is to be rated on each factor. These factors are indicated on the FD-17C and are to be interpreted according to the following definitions. These definitions should be kept by the rater and constantly referred to in completing the FD-17C. Unknown or Not Applicable (N/A) This rating is to be given to the member whom you have had no opportunity to observe in reference to the particular trait; or, the particular trait is not applicable to the member being rated. If this rating is chosen, check the column titled N/A which corresponds with the criterion being rated. Below Standard This rating is to be given to a member who does not meet the standard. A “Below Standard” rating must be addressed by a Performance Improvement Plan. Standard This rating is to be given to a member who consistently meets the requirements of the position in the trait being considered. Section 106.034d Page 3 of 9 Above Standard This rating is to be given to the member who consistently meets, and sometimes exceeds, the requirements of the position. An “Above Standard” rating must be addressed in evaluator’s comments. PERFORMANCE IMPROVEMENT PLAN The purpose of the Performance Improvement Plan (PIP) is to provide the probationary member a thorough understanding of his/her deficiencies and the expected performance standards necessary to facilitate improvement. Expected performance standards include, but are not limited to, FFD rules, regulations, policies, procedures, and/or supervisor expectations. The plan is to remain in effect until the next rating period. The PIP relies on the following three methodologies: 1. Education and Training It is agreed upon by members assigned to the PIP that the probationary member is to be provided the necessary training to meet the required standards. 2. Responsibility The probationary member is to accept the responsibilities listed. The supervisor is to take on the added responsibility ensuring compliance of the deficiencies listed. 3. Supervision The assigned supervisor is the primary supervisor for the probationary member. It is [his/her] responsibility to ensure the requirements of the PIP are met. The assigned supervisor is to document the result of monthly evaluations and PIP to be included as part of the member’s Department Personnel File. The documentation should outline compliance and any noncompliance issues related to probationary performance based on the performance expectations noted below. Section 106.034d Page 4 of 9 The intent of the PIP is to allow for self-accountability with checks and balances in place to ensure compliance of probationary performance. The plan provides the probationary member every opportunity to be successful while working for FFD. It is the probationary member’s ultimate responsibility to ensure he/she meets all performance expectations. Performance History Reflecting Need for Improvement Identify and list areas which are “Below Standard” on the Probationary Evaluation Form. Cite the policy or requirement and plan of action recommended to bring the member to Standard. Critical Incident or Safety Violation Identify and list events or violations occurring during the probationary month which would be considered critical incidents or safety violations. Cite the policy or requirement and plan of action recommended to avoid in the future. The listed factors indicate a problem(s) which needs to be rectified immediately. To facilitate improvement, the PIP is to be implemented to assist the probationary member in complying with FFD rules, regulations, policies, procedures, and/or supervisor expectations.
