OPERATIONAL POLICY This section intentionally left blank. OPERATIONAL GUIDELINE Company officers who may identify specific hazards at a premise or occupancy which would present significant challenges to mitigating potential incidents should utilize the FD-12 to document the locations of priority target hazards. PROCESS 1. List on the form Station, Shift, and Company Officer submitting the information. 2. Identify whether the location is an addition or deletion of priority target hazard’s location. 3. List on the form the Occupancy Number, Address, doing business as (DBA), and the reason for adding the location to the list. 4. Route to the battalion chief responsible for the Target Hazard Program.
