OPERATIONAL POLICY It is Department policy to investigate all accidents that result in serious injury, death, or significant property damage, with the goal of preventing and/or reducing the number and severity of injuries and accidents. INFORMATION The Department will operate an Injury and Accident Review Committee to investigate all accidents that result in serious injury, death, or significant property damage. The basic function of the committee will be to: Investigate, document, and report on all pertinent accidents. Work in coordination with other standing committees which may be of assistance in the investigation (e.g., Safety Sub-Committee and Investigations). Work in coordination with outside agencies (i.e., OSHA, Fresno PD, CHP, etc.). Meet quarterly to review investigation issues. Provide final disposition on investigations to the Fire Chief or designee. Injury and Accident Review Committee Members The Injury and Accident Review Committee chairperson will be the Safety and Wellness Officer. Reports are to be routed through the Training and Operations Deputy Chief to the Fire Chief. The committee will consist of representatives as follows: Safety and Wellness Officer (Chairperson) One representative from Local 202 One representative from FFD Training Division One representative from Fleet Management One representative from FFD Administration DEFINITIONS This section intentionally left blank.
