Johnson, Senior Administrative Clerk Section 204.013 Page 1 of 2 PURPOSE The purpose of this policy is to establish a procedure for reporting to Occupational Safety and Health Agency (OSHA) serious injuries, illnesses or if death occurs at the workplace. APPLICATION This section intentionally left blank. OPERATIONAL POLICY Whenever a state, county, local fire or police agency responds to an accident involving a member covered by OSHA in which a serious injury, illness, or death occurs, the nearest office of the Division of OSHA will be notified within eight hours by telephone by the responding agency (California Occupational Safety and Health Regulations, Chapter 3.2, Subchapter 2, Article 3, Section 342 (b)). PROCESS Company officers should ensure the following information is provided to Communications by cellular phone from the accident scene or by telephone from the fire station immediately upon return to quarters. Communications should be instructed to forward such information to Cal/OSHA. Section 204.013 Page 2 of 2 1. Time and date of accident. 2. Employer’s name, address, and telephone number. 3. Name, job title, and badge number of person reporting the accident. 4. Address of accident site or event. 5. Name of person to contact at site or event. 6. Name and address of injured member(s). 7. Nature of injury. 8. Location where injured member(s) was moved to. 9. List and identity of law enforcement agencies present at the site of accident. 10. Description of accident and whether the accident scene has been altered. INFORMATION Types of incidents that may be encompassed in this standard are transportation vehicle accidents, industrial accidents, or hazardous materials exposures.
