SECTION 106.050a UNIT 5 CERTIFICATION TRACKING FORM
PURPOSE
This document outlines the process for requesting and receiving Certificate Premium
Pay.
APPLICATION
The Unit 5 Certification Tracking Form was created to assist FFD members in
identifying and accumulating the necessary certificates to finish the process necessary
to obtain Certificate Premium Pay.
OPERATIONAL POLICY
Completion of all steps of the process outlined in this policy is required for a member
to receive Certificate Premium Pay. This process has been developed to ensure all
members eligible for certificate premium pay understand the process for receiving
compensation.
This policy applies to all members, including members hired into the Trainee
Academy.

PROCESS
Certificate Premium Pay:
1. Complete all requirements for the desired certificate premium pay.
2. Record a self-assignment in VectorSolutions Learning Management System
(LMS):
a. Complete the activity for the completed course.

b. Do this for all courses required for the desired certificate premium pay.
c. Ensure that all required certifications are uploaded into the LMS.
3. Send a completed and signed copy of the FD-50 to the Training Division for
processing.
4. Training Division:
a. Verify the completion of all requirements for desired certificate
premium pay.
b. Verify all documentation that has been uploaded to LMS.
c. Approve and send the FD-50 to payroll.
5. Fire Payroll:
a. Verify and complete payment requirements.
b. E-mail a signed copy of FD-50 to the member verifying the completion
of process and that employee will be receiving payment on the next
eligible paycheck.

a. Operations Certificate Premium Pay (must have all the following
certifications)
Ck Box
1. Hazardous Materials First Responder Operational
2. Firefighter I/II Certificates
3. Structural Collapse Specialist 1 Certificate
4. Confined Space Operations Certificate
5. Driver Operator IA/IB Certificates
b. Certificate Premium Pay (must have one of the following certifications) Ck Box
1. Employees holding a Fire Officer or Company Officer Certificate
2. Fire Instructor I, Training Instructor, Instructor 1, or Fire and
Emergency Services Instructor 1 Certificate
3. Fire Investigator I Certificate
c. Special Certificate Premium Pay (must have one of the following
certifications)
Ck Box
1. Employees holding a Fire Instructor III or Fire and Emergency
Services Instructor 3 Certificate
2. Training Officer Certificate
3. Fire Investigator II Certificate and/or Fire-Arson Investigator
Certificate
d. Chief Officer/Executive Fire Officer Certificate (must have one of the
following certifications)
Ck Box
1. Executive Fire Officer Certificate through the National Fire Academy
2. Chief Officer Certificate, Commission on Professional Credentialing
3. Educational Requirements for CA SFM, Chief Fire Officer Certificate
